Care Fund serves Arizona families of children with extended and critical illness or injury by providing rent or mortgage payment support. The following guidelines provide specific criteria to be eligible to receive financial assistance and are subject to change. Approval is on a case-by-case basis and is based on available funds.
FAMILY ELIGIBILITY REQUIREMENTS:
The applying family (parent/legal guardian) must...

Reside in the state of Arizona ANDis a U.S. citizen.

Have a child up to 18 years of age who is critically ill or injured with an extended illness/injury AND current extended hospitalization stay.

Certify the illness or injury by a medical care provider, social worker or hospital administrator.

Provide evidence of financial hardship.

Hold the mortgage of the home where the child resides or be the primary leaseholder on the rental agreement/lease. Payments must be up-to-date and not in a pre-closure status, Care Fund is unable to make back/late payments.

Provide the current month/year mortgage statement or unexpired lease agreement (only the primary residence qualifies).
CARE FUND'S PROMISE:
Care Fund will...

review each application on a case-by-case basis, placing priority on first-time applicants.

verify application information; applications with false information will be denied.

give a final determination of each family’s application regardless of race, religion or gender and based on available funds.

determine the length of time each family’s rent or mortgage expenses may be covered, if approved.

pay mortgage/rent directly to the mortgage holder/landlord.

define a housing expense payment as one of the following: (1) One month’s rent, as indicated on an executed lease agreement, inclusive of any applicable sales taxes, if listed on the provided rental agreement/lease, or (2) One monthly mortgage payment, inclusive of principal, interest, taxes, insurance, or mortgage insurance, if included on the provided monthly mortgage statement.

consider additional awards after 12 months or if a child with a chronic illness has a significant change in health status.

deviate from these guidelines upon agreement of the Care Fund Chairman of the Board, Founder or Executive Director.
